Following CDC guidelines, all patients, visitors, employees, and physicians entering the clinic will be screened by having their temperature taken and answering the following questions if they have a temperature of 100.0°F. If asked to go home the patient's appointment will be rescheduled.

SAFETY PROTOCOL

*Please note that new patients must check-in at the main APMI front desk located inside Suite 1600. Avesta Ketamine and Wellness is a partner of APMI Wellness Center. While Avesta is located inside Suite 1675, in order to best manage patient flow and conduct COVID-19 screening and temperature checks, we are asking all patients to check-in at the main desk located in Suite 1600. Thank you for your understanding and cooperation.

Mask and Guest Policy

 

Masks and Face shields

ALL patients & visitors are required to wear a MASK upon entering the clinic. Once in your treatment room and the staff have started your IV and checked your vital signs, you may take your mask off. Once your mask is off, please refrain from talking.

PLEASE NOTE: we are no longer allowing our employees, patients and visitors to wear a face covering or mask with a pop-off valve. This type of mask allows exhaled air to escape unfiltered to those around the wearer. If you arrive at our office with a pop-off valve mask, you will be offered a paper mask to cover your mask or replace your mask with the paper mask.

Face shields do not give enough protection for the person wearing them or the people nearby if they are worn alone. Face shields cannot be used instead of a mask. You may wear a face shield in addition to your mask, if you choose.

 

Guests

You can bring one additional guest to your treatment with you as you will need a ride home. We ask that the guest stay in the treatment area with you and keep their masks on at ALL times.

  • Have you had a fever of 100.4°F/38°C in the past three days?

  • Do you have a cough, sneezing, sore throat, headache, shortness of breath, difficulty breathing or headache?

  • Have you been in close contact (less than six feet away for 15 minutes or more) with anyone who

    • Has coronavirus

    • Is suspected of having coronavirus

    • Has been recently tested and is awaiting results or is suspected of having the coronavirus?

    • Have you tested positive for coronavirus in the past 14 days?
